Autism Awareness Enrollment

The Weber County Sheriff's Office Autism Awareness Enrollment is meant to assist law enforcement, EMS, Fire, and other emergency first responders with engaging and communicating effectively with individuals with autism. This voluntary enrollment form provides crucial information for emergency personnel responding to a scene or contacting an individual.

Completed and submitted forms will be entered into the Weber County Sheriff's Office Records Management System and are accessible by Weber County first responders and the Weber County Dispatch Center. It is recommended that this form be updated yearly, so first responders have current information when handling emergency and non-emergency situations.
