NEW FORMAT FOR RAMP APPLICATIONS (link below)

Please note that the application form for RAMP funding has changed. Instead of multiple forms for the different types of RAMP funding requests, there is now one form that may be used for the different RAMP categories. The form may be filled out online and printed or you can print out the blank format and fill it in. The information sheet that is attached to the application will help answer questions you may have about filling out the application and the type of documentation that is required. Remember that we will need 15 copies of the application and supporting documentation submitted with each request. Be sure to use the checklist on page 2 of the application to make sure you have met the filing requirement.
