What to DO when you are injured on the job:

  1. Report the Injury Immediately
    Notify your supervisor as soon as the injury occurs. Weber County is required to file a report of injury within 24 hours.
  2. Submit the Employee Incident Report
    You must complete and submit the 1st Report of Incident Form within 24 hours of the injury. Click the link to get the Employee Incident Report: Employee Incident Form
  3. Supervisor's Responsibility
    The supervisor of the employee must submit the Accident Investigation Report within 24 hours of injury. Click the link to get the Accident Investigation Report: Supervisor Accident Investigation Form
